New patients:

• Must begin with an initial exam. There are four options: 1-issue, 2+ issue, forearm-only, TMJ-only initial exams.

• Check your email for the consent form. Please fill out at your earliest convenience and in full detail.

• The Chiro Lab is out of network with all insurances and is a cash-based practice.

• We do not currently accept Medicare patients, even if you are willing to pay cash. Chiropractors cannot opt out of Medicare.
